Excel Award Nomination Form

The Excel Award consists of an award suitable for display on the staff member’s desk or bookshelf. Achievement of this award will be announced at a time determined by the employee and supervisor to most effectively highlight the employee’s efforts. The awards will be announced publicly on a semi-annual basis for those staff members receiving this honor. The Excel Award is given to a staff member in recognition of extraordinary performance by a staff member, performance above and beyond the call of duty, or exemplary performance recognized by a supervisor, colleague, partner, or University.
